I accept the quote, I have read and understood the terms and conditions of hire of Cougar Scaffolding Limited T/A Cougar Scaffolding 2020 Ltd, which form part of and are intended to be read in conjunction with this quotation form and agree to be bound by these conditions. I agree to settle my account in full, 7 days after the date of the invoice, or by prior written agreement with Cougar Scaffolding. I will organise requirements as noted on the quote, in the “requirements to be organised by the client” section and advise Cougar Scaffolding they are completed prior to scaffolding being set up or dismantled and I will provide any documentation required accordingly, especially for Health & Safety purposes. I authorise the use of my personal information as detailed in the Privacy Act clause therein. I agree that if I am a director / shareholder (owning at least 15% of the shares) of the Client I shall be personally liable for the performance of the Client’s obligations under this contract. I certify that the information supplied is true and correct.
